Overview

This presentation offers an in-depth exploration of advance care planning (ACP), serious illness communication (SIC), and the critical importance of addressing these topics in patient care. Through a comprehensive review of concepts, barriers, frameworks, and practical strategies, participants will gain insights into optimizing patient-centered care near the end of life.

Target Audience

This CME initiative was designed for oncologists, surgeons, radiation oncologists, NPs, PAs, pharmacists, nurses, and other healthcare professionals involved in the treatment of patients with cancer.

Melanie Smith, MD 

Melanie Smith, MD is an Assistant Professor of Medicine at Northwestern University Feinberg School of Medicine. She received her medical degree from Northwestern and completed residency in internal medicine at NYU. She returned to Northwestern for her fellowship in Hospice and Palliative Medicine and stayed on as faculty where she practices palliative care, focusing on co-management in the outpatient oncology setting and communication skills training.
